You didn't mention how much sun your yard gets, what kind of trees nearby, what kind of soil, drainage, or watering habits. All of these can be problematic.
How deep are you aerating? Is it possible you have a serious compaction just below the root level?
Why not try a year without anything being applied? Maybe just use a hose end sprayer soon, just for a splash of fertilizer and some weed control. More is not better...
